Waiver *

Please read and agree to the athlete waiver in order to complete your registration or free trial. For athletes under 18, the waiver must be completed and submitted by a parent or guardian. Athletes must be 12 and older to attend. Registration or free trial is not complete without your agreement. I, the parent/guardian/participant, in registering for Modernday Fitness Programs, understand that in attending the training/mentorship program and using the facilities does so at their own risk. Modernday Fitness, satellite locations, and their owners,employees and agents, shall not be liable for any damages whatsoever arising from any personal injury , property loss , or death sustained by the participant and their family in or about any programs on the premise of satellite workout sites, or any MDF sanctioned event i.e. football or college camps where travel is involved. I understand that indoor/outdoor training and mentorship is a physical and mental process which can require considerable running, starting, stoping, physical and mental exertion: and could potentially lead to limb injuries, possible permanent disability and death. Participants assume full responsibility for all injuries and damages which may occur in or about any programs on the premises and you do hereby fully and forever release, discharge, and hold harmless Modernday Fitness, all associated facilities and its/their owners, employees and agents from any and all claims, demands, damages, rights of action, present or future resulting from or arising out of any person’s participation in any programs of use of its facilities. In addition, he/she/I agree to follow the rules of play and conduct set by Modernday. He/She/I understand that failure to do so may result in suspension from participation. Also, he/she/I waive(s) all rights to any photos taken for use in any Modernday Fitness publication.
